In these cases, limit your assessment to level of consciousness LOCmotor strength, and pupillary reactivity but add other assessments that may yield important data based on your patient's clinical presentation. Any change in your patient's neurologic assessment, no matter how slight, requires immediate action. A brief neurologic assessment, including LOC, vital signs, pupils, cranial nerves, and motor function, should be performed.
